comprehensive health care clinic (CHCC)
Medical facility that provides outpatient and ambulatory care services with a limited holding bed capacity.

Total coverage for health care-related charges. This coverage is after deductibles and co-insurance are applied to any hospital or physician’s office visit costs.
comprehensive health planning (CHP)
Health planning initiative of the Comprehensive Health Planning and Public Health Services Amendment of 1966 and replaced by the National Health Planning and Resources Development Act of 1974. It was intended to encompass all factors and programs affecting the health of people in the United States. Federal support for CHP was eliminated in 1986.
Comprehensive history
In 1995 the American Medical Association (AMA) and Centers for Medicare and Medicaid Services (CMS) developed documentation guidelines for CPT evaluation and management services and modified them in 1997. For a comprehensive history, medical documentation must include the chief complaint, an extended history of present illness (HPI), a complete review of systems (ROS), and complete past, family, and/or social history (PFSH).
