A certificate of insurance is evidence that a person or business has certain types of insurance. In order to be valid, the certificate must be countersigned by the agent-of-record. Certificates indicate the broad general types of coverage the insured has along with the limits of coverage but anyone requesting a certificate of insurance needs to inquire further about any possible limitations of coverage. An advantage of a certificate of insurance over just a copy of the policy is that certificate holders receive copies of any notices of cancellation that are sent to the insured.As an example, suppose ABC Cleaners rents space in a strip shopping center and is required by the landlord to carry $100,000 of Commercial General Liability coverage. In order to prove to the landlord that the insurance is in force at the requested limits, ABC Cleaners’ agent would issue a certificate of insurance. (See Agent of Record; Countersignature).

(b) A document that is issued by a coverholder which evidences the existence of insurance cover and details the terms of such cover. No policy of insurance is issued where such a certificate is issued.

Certificate of Insurance, Motor, Automatic Transfer of
As per provisions of Sec. 157 of the New Act (1988) where a person in whose favor a certificate of Insurance has been issued, transfer to another person the ownership of the motor vehicle in respect of which the Insurance was taken, then the certificate of Insurance and the relative Policy in so far as the Compulsory Act Insurance portion is concerned shall be automatically deemed to be transferred in favor of the new owner from the date of transfer of ownership of the vehicle. However own damage Insurance portion is not automatically transferred.
Certificate of Manufacture
A Statement in which a producer specifies where his goods were manufactured, certifies that manufacturing has been completed, and confirms that the goods are at the buyer’s disposal.
Certificate of medical necessity (CMN)
1. Certification by a provider to an insurance carrier that a medical service or procedure is medically necessary because of the patient’s condition. 2. Form required by Medicare that allows the patient to use certain durable medical equipment prescribed by a physician or one of the physician’s staff. 3. In Medicare fraud, to complete a CMN for patients not personally and professionally known by the physician.
Certificate of merit
Document that is required for an independent medical expert to review a patient’s medical record and certify that a claim has merit before a formal medical liability lawsuit is filed.
Certificate of need (CON)
Document of approval issued by a state or government agency that legally allows an organization or health care institution to construct or modify a health care facility, incur a major capital expenditure, obtain expensive medical equipment, or offer a new or different health service. An investigation is done before receiving the CON to make sure the facility or service meets the needs of those for whom it is intended. Also referred to as planning approval.
Certificate of Origin
Documentary evidence to the import country’s customs authorities that the cargo: (a) is not from an embargoed or restricted country; (b) genuinely comes from the country specified; (c) is included in official statistics; (d) is included or may be included with other documents.

Certificate of waiver
Document issued to a laboratory to perform only waived tests. These are simple examinations or procedures that use methodologies that are so easy and accurate that the likelihood of erroneous results is negligible and poses no reasonable risk of harm to the patient if the test is performed incorrectly.
