1. Month, day, and year that the incident or exposure occurred. 2. In workers’ compensation, months, day, and year the employee first suffered disability from an accident or exposure by present or previous employment. Many times, the date of injury may be the last day worked. When completing the CMS-1500, the date of injury is inserted in Block 14 of the paper claim form.

When an insurance claim is manually submitted, the date of service is inserted in Block 24A of the CMS-1500 insurance claim form.
