(ii) An individual entry, such a piece of jewellery, listed with its description and valuation on a schedule by a policy showing items covered.

Detailed invoice that lists charges, payments, adjustments, and total balance due. Also referred to as billing statement or summary of charges .
Itemized statement
Detailed summary of all transactions of a patient’s account (i.e., dates of service, description of services, number of services provided, noncovered services, amount charged, payments [copayments and deductibles], date the insurance claim was submitted, adjustments, and account balance). Itemized statements are generated by physicians and hospital facilities after services have been provided to patients. Also called itemized bill .
