A statement or declaration reduced to Writing and sworn to or affirmed before some officer who has authority to administer oaths or affirmations.

A form required when a claim is filed. In general, it contains the facts on which the claim is based.
Affiliate
Corporation in which a majority of the stock is owned by any or all of the stockholders, directors, or officers of another corporation. These individuals also own a majority of the stock in that other corporation.

Companies related through common ownership of their securities or interlocking Directorates.
Affiliated contractor (AC)
Medicare carrier, fiscal intermediary (FI), or other contractor such as a durable medical equipment regional carrier (DMERC) that shares some or all of the provider service carrier’s (PSC’s) jurisdiction in which the affiliated contractor performs non-PSC Medicare functions such as claims processing or education.
Affiliated entities
Under the privacy rule, allows several entities to be treated as a single covered entity. Only one consent is necessary, which binds all of the affiliated entities.
