Phantom driver
When an insured hits an object but reports it to the carrier as a hit and run by a phantom vehicle the insured has little description of the vehicle and there are never any witnesses.
Phantom vessel
Typically a vessel that has no valid classification, i.e. it is not registered with any recognised ship registry and is used as a vehicle for fraud on cargo owners. In Nima Sarl v. Deves Insurance Co. Ltd (2002), the Court of Appeal disallowed the insured’s claim as the vessel, considered by the insurer to be a phantom, never intended to sail for the destination named in the policy. The risk did not attach.

Practice in which the pharmaceutical care practitioner takes responsibility for all of a patient’s drug-related needs and is held accountable for this commitment. It is care that a patient requires and receives, which ensures safe and rational drug usage.
Pharmaceutical care practitioner
Individual who renders beneficial drug therapy management services and creates a covenantal bond or therapeutic relationship between the patient and pharmacist.

Pharmacy and therapeutics committee (Health Insurance)
Sometimes referred to as a P&T committee. A committee comprised of doctors who provide advice to the health plan regarding prescription drugs and their use.

Panel of multispecialty physicians who advise the managed care plan on safe and effective prescription drugs and develop and maintain a drug formulary.
pharmacy benefit manager (PBM)
Company under contract with managed care organizations, self-insured companies, and government programs to manage pharmacy network management, drug utilization review, outcomes management, and disease management. The goal is to save money. For example, a pharmacy benefit manager may fill drug prescriptions by mail order as part of a corporate health insurance plan. Also called a pharmacy benefit management organization or prescription benefit manager (PBM).
